First Visit
Brief Paperwork
Upon entering the clinic, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition.
Save yourself some time download our two intake forms, print them off and complete them being as detailed as possible. Then bring them along to your first appointment.
Patient history form. Please carefully fill this out and shade in areas of complaint on the pain diagram. Then indicate whether you consent or not to us contacting your GP should we need to. Please be thorough as approximately one in 15 people that we see we need to refer back to their GP because of complicating health reasons. We need to make sure that it's safe to treat you.
Baseline outcome questionnaire form. If we don't know where we have been we don't know where we are going. This gives us a better understanding of how your pain is affecting your quality of life. We do these at regular intervals to measure how your are improving.
Consultation
Once the paperwork has been completed, you will have a consultation with the chiropractor to discuss your health-related problems, concerns, as well as potential treatment options.
History and Examination
In order to determine what your actual problem is, the chiropractor will ask you various questions related to your condition. Next, a number of specialised tests will be performed to determine which tissues are affected and to what extent.
X-ray or MRI Studies
Your specific condition may occasionally require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.
Same Day Treatment
Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal manipulation, and/or soft tissue therapies.
Home Instructions
Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.
